“More U.S. health care dollars are spent treating back and neck pain than almost any other medical condition, but much of that money may be wasted, a new study suggests. Researchers from the University of Washington, Seattle, found that the nation’s dramatic rise in expenditures for the diagnosis and treatment of back and neck problems has not led to expected improvements in patient health.”

“Overall, pharmaceutical expenditures related to back and neck pain increased by 188% between 1997 and 2005, but cost associated with prescription narcotics rose by a whopping 423%.”

“University of Washington Medical Center clinician Richard Deyo, MD, MPH, tells WebMD that increasing use of the newer narcotics such as the drugs Vicodin and OxyContin is largely responsible for the increase, even though their use for chronic pain is controversial. He says patients need to recognize that the drugs have MANY potential side effects, including drowsiness, persistent constipation, and sexual dysfunction.’

‘Deyo co-directs the University of Washington Center for Cost and Outcomes Research, and he was a co-author of the study. “We still don’t know much about their long-term efficacy and safety for chronic back pain,” he says. “PATIENTS NEED TO UNDERSTAND that if they take these (opioid) medications long term, after a few months it will be DIFFICULT to stop. And there is pretty good evidence that long-term use may actually INCREASE SENSITIVITY to PAIN.” (1)

Alarming! I should say so! Chronic back pain! It bothers you from minute to minute, day to day, week to week, month to month and sometimes even year to year. You have tried this, and tried that. Some of you may have found a solution. Others are still searching. Some of you are using allopathic approaches. Others holistic and homeopathic approaches. Yet, some of you are combining all approaches. Whatever your particular [ this ] or [ that ] chronic pain is quite real and affects your daily life and those around you too.

Does everyone have chronic back pain? Thank goodness, no. However, the majority of us will experience this at some point in our lives and unfortunately it appears more frequently as we age. What causes this problem? Well other than, genetics which can be attributed to degenerative aspects, osteoporosis, arthritis and spinal stenosis to name just a few or some other serious disease such as arthritis or diabetes we usually inflict it upon ourselves. We neglect excercises that can strengthen our back and abdomen muscles to minimize the chance of injury or we do too much too soon. We lift something the wrong way something that is too heavy causing spraining, straining, and spasms, in our back. For kids it could be back packs with too many books in them. Obesity, pregnancy, bad posture, sleeping the wrong way also puts unwanted stress on our spinal columns. The pain associated with these last ones usually go away once the weight is lost, the child is born, posture or sleeping position is corrected.

For the other serious issues, a pinched nerve, ruptured or compressed spinal discs put some type of disruption on the transmissions of signals that assist our locomotion thereby causing pain. Our disc start losing fluid with age. That fluid acts as a shock absorber and helps with flexibility. Think about the times that you might have stepped off of a curb without paying attention. Do you rememer the shock that was transmitted through your back? That fluid helped to absorb and spread the force more evenly to minimize what you did. The lessening or abscense of that fluid would have inflicted much greater pain and longer lasting too.

Though it may not make you feel better, you are not alone in your pain or desperation to find a solution to either eliminate it or diminish it. 80 plus billion dollars per year is spent dealing with this problem in the United States. Do you realize what that number represents? Let’s put it this way, we are spending as much on back pain issues as we do on cancer and arthritis combined. And the results are UNDERWHELMING.

You should be aware by now that most if not all allopathic approaches have side effects like allergic reactions, depression, drowsiness, judgment impairment, decreased reaction time, and potential for addiction, and even increasing pain. So ingesting these opioids, analgesics (ingested or topical), anticonvulsants or products that contain salicylates, which some people may have a high sensitivity to, may give you temporary relief but present other problems for your body. Apparently though, these down sides have not stopped nor slowed the pace for either these things being prescribed or us asking for them.

Unless it has been determined that surgery is necessary, chiropractic and acupuncture have been shown to be effective. Cold compresses as well as hot ones and very warm baths seem to bring relief too. Exercises that strengthen the abdomen and back muscles such as stretching exercises, yoga swimming, walking, and correct posture all can and do help improve or minimize back pain.

Ah! Did that pique your interest? I sure the hell hope so! Especially, pregnant women and parents with children in the developing stages of life. Listen up! No [carefully] peruse the contents of this blog to the end. You won’t be able to say that you didn’t know. You just might decide to take action. And, you will find the missing letters.

First a very brief history of wireless. Way back around the very early 20th century AT&T was thinking about bringing wireless communication to the public. It was shelved because they felt it would cannabalize their monopoly on wired communication hardware and services. Also, given the infancy of wireless technology of that time you can imagine the size of the equipment,

Three quarters of a century later the first mobile device used extensively by Captain James T. Kirk made its debut on American television, well almost. Actually the time frame is about right but not the actual inventor nor place it debuted. American Martin Cooper was the inventor and this first mobile device was introduced to the world in Japan. The cost was quite prohibited, $3,500.00US. Then in 1996 Nokia introduced the precursor to our modern day smart phones. Though mobile devices have grown tremendously throughout the world “smartphone” growth was much slower until very recently. It has been estimated that from 1996 until 2011 “smartphone” growth was about three quarters of a billion users. By 2014 another 1 billion people will be using these! 1 billion new users inside of 3 years instead of almost 15 years! 2 billion of us of the 7 billion plus people will be walking around with these devices! More people than ever will be tied to these devices as if it were a life giving umbilical cord. We will use them for communication by all the means currently available of course and maybe some not seen. They will continue to be fashion statements, status symbols, they are thought to be will sexy. We will continue to bling bling them (see I made that into a verb) and add all the personal individuality to them as — well there are people. We are now, and will continue giving them to our very young progeny (as baby sitting devices). And, we will continue putting them next to our ears and talk incessantly (business people and kids), in our front pockets (men), in our back pockets and bras (women). We continue sleeping with them next to our heads while on (kids and teenagers). We will continue to be tethered, physically or wirelessly by our headsets and bluetooth connectors.

BUT, the one thing we 2 billion plus people are not doing is PROTECTING ourselves from the very thing we find such a necessary part of our lives! These devices emit Radio Frequencies that are considered low level micro waves. Supposedly, not at a level to cause biological harm to humans.(1)

I used the word supposedly for this very reason — almost all manufacturer’s of cell phones print somewhere in their manuals a warning of sorts. It is not explicit like saying WARNING. But, it is written in a way that definitely will draw minimal attention to this very important issue that — caution should be taken as to how close such devices should be placed to the body. So, by now a QUESTION should be moving through your brain, WHY? Many in the cell phone industry might answer with this response – ‘Do not worry because our products meet FCC (Federal Communication Commission) & radiation emission guidelines on SAR (Specific Absorbtion Rate)’. Well, these guidelines are just that – guidelines. According to their own statements a ‘single SAR value does not provide sufficient information about the amount of RF exposure under typical usage conditions to reliably compare individual cell phone models.'(2)

I could definitely go into a more detailed discussion of what is beginning to be brought to light as to the possible adverse physiological affects of this burgeoning new technology upon us. I could also could get into a more technical discussion, but for now I will reserve that for another blog. The purpose of this blog is to create an awareness that will allow you to take some type of action to PROTECT yourselves and especially your progeny. (Just in case)

Before I leave you to ponder what I have written, here are some statements by two individuals looking into this topic. Dr. Nora Volkow, the lead author of the JAMA study, said modeling studies have shown that a child’s brain would absorb more radiation than those of adults. ( Some studies have shown that radiation absorbtion rate for children’s brains is twice as much compared to adults because of children’s thinner skulls and because their brains have higher water content). But Volkow said she’s unaware of studies that have looked at the different developmental effects. Some studies have suggested that the radiation can cross or affect the entire brain of children, who are still developing, versus a particular area in adults,” said Dr. Stephanie Wagner, co-medical director of the neuro-oncology program at Indiana University Health and the IU Simon Cancer Center in Indianapolis. But studies looking at the effects of electromagnetic radiation in children and the risk of epilepsy and behavioral problems such as attention deficit disorder and aggressive behavior showed conflicting results, Wagner said. Still, why take a chance, Volkow said. She suggested that parents “teach their children to use their cell phones with a wired earphone and/or use the speakerphone mode and to avoid putting their cell phones directly on their ear.” (3)

We parents, educators, and adults today no longer talk of sex with our youth in terms of “the birds and bees”. We openly discuss sex and the responsibilities that can go along with that including protection. The “c _ _ _ _ m” (4) is one device that can afford protection. So too in this area of cell phone EMF ( electro magnetic frequencies), we should be openly educating as well as taking precautionary measures for ourselves and our children now and not later. Is there a “c _ _ _ _ m” (4) for your mobile device? I don’t know and maybe not in the literal sense of the word. However various precautionary means are available now that we can all do as we see, hear and discover more information bearing light on this topic concerning our health.

Welcome to this final treatment on the topic of Primum No Nocere. What a journey it is and it is not over. The basic tenent of First, Do No Harm has survived the millennia. Our world has become smaller and more inter-connected because of technology. Because of this lines have blurred between cultures, societies, male and female as to what we are capable of and what is expected. This has caused a transfer of information at unprecedented rates. We are finding out that we truly are much more alike than different, despite differences. As this transfer of information has grown our understanding of ourselves and of the workings of anatomy has grown many-fold. These progressions of the human, mind, body and spirit has also caused a change to the what we write. They are reflected in the progression of these medical oathes.

It is apparent that deities no longer influence many of these thought processes as they did in Hammurabi’s,

Vaidya’s, Hippocrates’, Sun Simiao’s Asaph’s, Maimodes’ and Enjuin’s time with their polytheistic and monotheistic overlays. (Though I believe that theism still plays an underlying role). What once was taboo is now passeé or on its way to be. The oaths have taken on a more humanistic form. You will see this change in the more current revised version of the Hippocratic oath and the AMA Code of Medical Ethics. And, surprisingly for me at least, these oathes are no longer a requirement of graduating physicians.(1) In some cases these oathes are now being called “declarations”.

The Seventeen Rules of Enjuin are a code of conduct developed for students of the Japanese Ri-shu school of medicine in the 16th century CE. These rules require physicians to work together with others of their trade as a brotherhood. The guidelines regarded conduct not only in action but also in mind, prohibiting immoral feelings when examining a woman.

1. Each person should follow the path designated by Heaven (Buddha, the Gods).
2. You should always be kind to people. You should always be devoted to loving people.
3. The teaching of Medicine should be restricted to selected persons.
4. You should not tell others what you are taught, regarding treatments without permission.
5. You should not establish association with doctors who do not belong to this school.
6. All the successors and descendants of the disciples of this school shall follow the teachers’ ways.
7. If any disciples cease the practice of Medicine, or, if successors are not found at the death of the disciple, all the medical books of this school should be returned to the School of Enjuin.
8. You should not kill living creatures, nor should you admire hunting or fishing.
9. In our school, teaching about poisons is prohibited, nor should you receive instructions about poisons from other physicians. Moreover, you should not give abortives to the people.
10. You should rescue even such patients as you dislike or hate. You should do virtuous acts, but in such a way that they do not become known to people. To do good deeds secretly is a mark of virtue.
11. You should not exhibit avarice and you must not strain to become famous. You should not rebuke or reprove a patient, even if he does not present you with money or goods in gratitude.
12. You should be delighted if, after treating a patient without success, the patient receives medicine from another physician, and is cured.
13. You should not speak ill of other physicians.
14. You should not tell what you have learned from the time you enter a woman’s room, and, moreover, you should not have obscene or immoral feelings when examining a woman.
15. Proper or not, you should not tell others what you have learned in lectures, or what you have learned about prescribing medicine.
16. You should not like undue extravagance. If you like such living, your avarice will increase, and you will lose the ability to be kind to others.
17. If you do not keep the rules and regulations of this school, then you will be cancelled as a disciple. In more severe cases, the punishment will be greater.

Sí, Se Puede. Yes We Can, What?

Sí, Se Puede, Spanish for yes we can. It is what a group of scientist said they could create when asked if it could be done. What were they asked? Read on and find out.

The ancient Chinese (yes current day Chinese too) understood that our world as well as ourselves were made up of energy. They deduced that our bodily functions are regulated by an energy too. They gave the name Qi (pronounced chee) to this invisible bodily energy that flows through our bodies via pathways called meridians. First, Do No Harm Part II – Primum Non Nocere

Primum Non Nocere, (First, Do No Harm). Part II of III.

Hopefully you have read part one of this blog, “First, do no harm”. It should have made you aware how entrenched this idea is in our human psyche. It is so innate and so powerful that it is cross cultural and has been used by the practitioners of the healing arts and sciences, past and present. You have seen this idea had its roots long before the Greeks and Hippocrates and it was important enough to not only be written down, but inscribed in stone. Much was centered around the diety of those ages or the queen, king or pharoah. And sometimes these earthly monarchs were considered both man and god despite their mortal shortcomings. Much was expected out of these ancient medical practitioners even though their true abilities would be considered quite limited by today’s standards.

There were severe repercussions in ancient times for messing up too. In ancient times the gods were supposed to have the final say in matters of health, life and death. In actuality those in power did. The practioners experienced the wrath of their queens, kings, pharoahs if their loved ones became more critically ill or died at the hands of these practitioners. The punishment could be as minor as imprisonment and a beating, to as severe as losing a bodily part or one’s life.

I will touch upon Sun Simiao, Asaph and Maimondes in this blog.

SUN SIMIAO OATH (581–682 CE)*

What Hippocrates is to the west Sun Simiao of China would be considered to the east. Simiao was also renown for the great care that he gave those he treated. Like Hippocrates, Simiao emphasized ethical and avoiding inappropriate behavior for physicians through his own practices and his writings. He wrote two books and a treatise that were just plain amazing for the time due to the scope and detail of information that accurately assisted patient and physician alike.

The two books were named “Beiji Qianjin Yaofang” (Prescriptions for Emergencies Worth a Thousand Gold ) and “Qianjin Yifang” which can be considered an addendum to the first. Plus he wrote the treatise “The Absolute Sincerity of Great Physicians”. The quote below gives you some idea of Simiao’s outlook and approach to his profession and what he expected of others.

‘A Great Physician should not pay attention to status, wealth or age; neither should he question whether the particular person is attractive or unattractive, whether he is an enemy or friend, whether he is a Chinese or a foreigner, or finally, whether he is uneducated or educated. He should meet everyone on equal grounds. He should always act as if he were thinking of his close relatives.’

ASAPH OATH**

Over in another part of the globe we find Asaph Judaeus. 55 rules or codes are attributed to this Jewish physician. . His work like Hippocrates is attributed to him though there are reservations in the academic world as to who actually did the writing. Asaph’s works are considered the oldest written Hebrew writings explaining physicians code of ethics.

MAIMONIDES OATH/PRAYER***

Moving forward in time, a little over 1000 years from about 550 CE to about 1500 CE we come across another influential man of medicine, Moses Maimonides/Mūsā ibn Maymūn of Cordoba (Spain)

“Grant that my patients have confidence in me and my art and follow my directions and my counsel.

• Remove from their midst all charlatans and the whole host of vicious relatives and know-all nurses, cruel people who arrogantly frustrate the wisest purposes of our art and often lead Thy creatures to their death.

• Should those who are wiser than I wish to improve and instruct me, let my soul gratefully follow their guidance; for vast is the extent of our art. Should conceited fools, however, censure me, then let love for my profession steel me against them, so that I remain steadfast without regard for age, for reputation, or for honor, because surrender would bring to Thy creatures sickness and death.

• Imbue my soul with gentleness and calmness when older colleagues, proud of their age, wish to displace me or to scorn me or disdainfully to teach me. May even this be of advantage to me, for they know many things of which I am ignorant, but let not their arrogance give me pain. For they are old and old age is not master of the passions. I also hope to attain old age upon this earth, before Thee, Almighty God!

• Let me be contented in everything except in the great science of my profession. Never allow the thought to arise in me that I have attained to sufficient knowledge, but vouchsafe to me the strength, the leisure and the ambition ever to extend my knowledge. For art is great, but the mind of man is ever expanding.

• Almighty God! Thou hast chosen me in Thy mercy to watch over the life and death of Thy creatures. I now apply myself to my profession. Support me in this great task so that it may benefit mankind, for without Thy help not even the least thing will succeed.”

I hope that you have found this very brief discussion at least enlightening if not exciting. It definitely shows how the medical profession has progressed and is progressing. We are discovering that our ancestors were more correct than even they may have realized or even we realized. Our communities are beginning to move towards approaches that are assisting our bodies to help itself without the fear of side-effects. This is only the beginning of this resurgence, but we are finally moving in the right direction. Our ability to compile centuries of medical knowledge is finally moving us back in a sense, towards more holistic and homeopathic approaches. We are now combining this with the best modern science has to offer.

The concept of “First, do no harm” in the practice of medicine is prevelant in virtually every society throughout humankind’s written history. Oaths were a form of a strong binding commitment within the medical profession. And, for many many years oaths were taken so much more seriously even to the point of dire consequences. When religions and kings were the ruling force in societies, those oaths were usually taken with the particular diety or king in mind. Sometimes they were one and the same. Either way they were a force one would have to reckon with if the oath were ever broken. If the diety didn’t get to you for malfeasance then your king did or you god/king did.

The United Stated for the most part is settled by those influenced by western ideologies and principles. Those roots come heavily from Greek and Roman influences. And so it is only fitting that as society has become more “civilized” it created a system of oaths and principles to hopefully improve our lot individually and communally.

In going through my formative educational years, the Hippocratic Oath was spoken of even outside of the medical profession. Hippocrates was able to influence the change from medicine being mystical and religious oriented and directed it towards the science of cause and effect. His ideas in the western world had such impact, that he is known as the “father of modern medicine”.

THE CODE of HAMMURABI

HAMMURABI’S tablet of almost 300 Babylonian laws is one the most renown in all of history. It inferred the concepts of Primum non nocere or “First, do no harm”. It also included rules for physicians of the day that may seem severe by today’s standards. I do wonder though would there be less malpractice and litigation if those rules were in place today ? Just wondering.

215. If a physician make a large incision with an operating knife and cure it, or if he open a tumor (over the eye) with an operating knife, and saves the eye, he shall receive ten shekels in money.
216. If the patient be a freed man, he receives five shekels.
217. If he be the slave of some one, his owner shall give the physician two shekels.
218. If a physician make a large incision with the operating knife, and kill him, or open a tumor with the operating knife, and cut out the eye, his hands shall be cut off.
219. If a physician make a large incision in the slave of a freed man, and kill him, he shall replace the slave with another slave.
220. If he had opened a tumor with the operating knife, and put out his eye, he shall pay half his value.
221. If a physician heal the broken bone or diseased soft part of a man, the patient shall pay the physician five shekels in money.
222. If he were a freed man he shall pay three shekels.
223. If he were a slave his owner shall pay the physician two shekels.

Around the 15th century BC it appears that privacy and ethical sexual conduct issues began being incorporated into a physician’s practice. This is seen in the The Indian Vaidya’s Oath.

VAIDYA’S OATH

This oath, was taken by Hindu physicians around the 15th century BCE.

• You must put behind you desire, anger, greed, folly, pride, egotism, jealousy,
harshness, calumny, falsehood, sloth, and improper conduct.
• With short-cut nails, ritually clean and clad in the orange garment, you must be .
pledged to truth, and full of reverence in addressing me …
• If, however, you behave perfectly, while I profess false views, I shall be guilty of sin
and my knowledge shall bear me no fruit.
• (after having finished your studies) with your medicaments you shall assist
Brahmins,venerable persons, poor people, women, ascetics, pious people seeking
your assistance, widows and orphans, and any one you meet on your errands, as if .
they were your own relatives. This will be right conduct …

Then Hippocrates appeared on the scene. He took medicine out of the mystic world and began applying scientific approaches of cause and affect.

HIPPOCRATIC OATH, ANCIENT

I swear by Apollo Physician and Asclepius and Hygieia and Panaceia and all the gods and goddesses, making them my witnesses, that I will fulfill according to my ability and judgment this oath and this covenant:

• To hold him who has taught me this art as equal to my parents
— to live my life in partnership with him.
— if he is in need of money to give him a share of mine,
— to regard his offspring as equal to my brothers in male lineage and to teach them this art, If they desire to
learn it without fee and covenant;
• to give a share of precepts and oral instruction and all the other learning to my sons
and to the sons of him who has instructed me and to pupils who have signed the covenant and have taken an oath
according to the medical law, but no one else.
• I will apply dietetic measures for the benefit of the sick according to my ability and judgment;
• I will keep them from harm and injustice.
• I will neither give a deadly drug to anybody who asked for it, nor will I make a suggestion to this effect.
• Similarly I will not give to a woman an abortive remedy. In purity and holiness I will guard my life and my art.
• I will not use the knife, not even on sufferers from stone, but will withdraw in favor of such men as are engaged in this work.
• Whatever houses I may visit, I will come for the benefit of the sick, remaining free of all intentional injustice, of all mischief and in particular of sexual relations with both female and male persons, be they free or slaves.
• What I may see or hear in the course of the treatment or even outside of the treatment in regard to the life of men, which on no account one must spread abroad, I will keep to myself, holding such things shameful to be spoken about.
• If I fulfill this oath and do not violate it, may it be granted to me to enjoy life and art, being honored with fame among all men for all time to come; if I transgress it and swear falsely, may the opposite of all this be my lot.
